For decades, pneumonia has remained one of the world’s deadliest infectious diseases, silently stealing the breath of millions and leaving devastated families and overwhelmed health workers in its wake.
In Malawi, where the burden of the disease continues to weigh heavily on vulnerable communities, a promising scientific breakthrough is offering renewed hope in the fight against the killer infection.
At the center of that hope is the Malawi Liverpool Wellcome Programme (MLW), where researchers are advancing trials for a new vaccine known as Pneu-Bio-Vax (PBV) — a development scientists believe could significantly reshape the battle against pneumonia.
Pneumonia, caused by bacteria, viruses, or fungi, attacks the lungs and makes breathing difficult, particularly among children and the elderly. In Malawi, the disease has long been one of the leading causes of death among children under the age of five.
According to UNICEF Malawi, nearly 49,000 children in the country could die from pneumonia over a decade if interventions are not strengthened. The disease disproportionately affects poor and marginalized communities where access to healthcare, proper nutrition, and clean air remains limited.
Despite the introduction of vaccines and continued public health interventions, pneumonia still accounts for nearly 30 percent of deaths among children in Malawi, highlighting the urgent need for more effective prevention measures.
Now, scientists at MLW believe the PBV vaccine could provide a broader and stronger shield against the disease.
MLW researcher Steve Mbewe explained that unlike the current PCV13 vaccine, which targets only a limited number of pneumococcal serotypes, the new vaccine is designed to offer protection regardless of serotype.
“There are over 100 serotypes responsible for causing pneumococcus or pneumonia, and PCV13 only targets certain serotypes,” said Mbewe. “That limitation is one of the reasons why we are still recording high cases of pneumonia.”
He said PBV is protein-based and targets proteins found in more than 90 percent of the bacteria currently responsible for causing pneumonia, potentially making it far more effective than existing vaccines.
The ongoing trial in Malawi involves 416 volunteers aged between 18 and 50 years. Participants undergo extensive medical screening, including HIV testing, pregnancy tests, assessments for chronic illnesses, nasal wash examinations for natural pneumococcal carriage, COVID-19 throat swabs, and full blood tests.
The vaccine has already shown promising results in an earlier trial conducted in the United Kingdom by the University of Liverpool. Researchers reported that PBV reduced pneumonia transmission by 35 percent — a performance scientists say is more encouraging compared to the current PCV13 vaccine.
For health advocates, the progress represents more than just a scientific milestone; it is a potential lifeline for thousands of Malawian families.
Health rights expert Dorothy Ngoma expressed optimism that the vaccine, once approved, could significantly reduce pneumonia cases in the country.
“Any vaccine that has been tested and found useful in reducing infections is something we should welcome,” she said. “I believe this vaccine has been carefully formulated and could make a real difference.”
Ngoma also commended researchers working tirelessly on the project.
“It is commendable for the team working day and night to make this possible. We support them, encourage them, and give them a thumbs up for the good work they are doing,” she said.
Globally, pneumonia remains the leading infectious killer, claiming approximately 2.5 million lives every year, including more than 600,000 children under the age of five.
Despite the availability of vaccines and treatment, a child still dies from pneumonia every 43 seconds, particularly in low- and middle-income countries where malnutrition, air pollution, and poor access to healthcare continue to fuel the crisis.
For Malawi, where hospitals continue to receive streams of children struggling to breathe, the development of PBV represents more than a medical experiment. It symbolizes the possibility of a future where fewer families lose loved ones to a preventable disease.
While researchers caution that more testing and approvals are still needed before the vaccine becomes publicly available, the progress marks an important step toward changing the course of pneumonia in Malawi and beyond.
For now, families, scientists, and health workers alike are holding onto hope that a breakthrough born in a laboratory could one day save countless lives
